He plays be sense of smell.
It's hard to see how anybody, even Tommy, could play pinball by sense of smell, but I suppose the high scores must speak for themselves. The fact that Tommy can't hear or see seems to play into his advantage, and he can't get put off by the flashing lights and loud noises of the local arcade.
